What to Expect

Pre-Registration + Registration + Admission

We respect your time and will do our best to make your admission process as smooth and efficient as possible. One of the ways is for you to come in and pre-register after your doctor has scheduled your surgery or lab tests.

For your convenience, you may print our “General Consent for Treatment” form in English or Spanish prior to coming to the hospital. This will give you additional time to review the document and help expedite your registration. Please present your signed copy upon arrival to our registration department.

When can you register:

You may register at your convenience prior to your appointment or on the day of your appointment or admission. To register prior to your appointment, please click on this Registration Form, print it out, and mail it to Rockdale Medical Center, 1412 Milstead Avenue, Conyers, GA 30012, ATT: PAS Department.

What should you bring when you register:

You must bring the following items with you when you register:
  • Photo ID
  • Current Insurance Cards
  • Physician’s Order (except for Emergency Department visits)
  • Payment for co-pays, deductibles, or deposits due

What should you bring when admitted:

You may want to bring personal convenience items with you such as toothpaste, toothbrush, slippers, pajamas and a bathrobe to make you feel more comfortable during your stay. Items such as photographs often make patients feel more at ease. Do not bring large sums of money or other valuables with you to the hospital. We cannot assume responsibility for them. After you are admitted, you will be shown your room and given a Patient Information Booklet with more detailed information about your hospital stay.
